Background: Organ and tissue donation is the final treatment option for patients with end-stage chronic kidney disease. The limited availability of donors results in long waiting lists. Objective: To analyze the perceptions and attitudes of patients on the kidney transplant and hemodialysis waiting lists regarding organ and tissue donation, and to identify factors associated with the decision to donate. Material and methods: An observational, cross-sectional study of patients with end-stage chronic kidney disease on hemodialysis and on the cadaveric kidney transplant waiting list at a tertiary hospital in Puebla, Mexico. The Factors Influencing Organ Donation (FIDO) questionnaire was administered to assess attitudes, knowledge, and willingness to donate, as well as sociodemographic and clinical variables. Descriptive statistics were used. Results: Forty patients were interviewed, 42.5% were women, with a median age of 46 years (29-62); 100% had a favorable attitude toward transplantation, but only 42.5% were willing to donate. Knowledge was moderate (70%) and low (22.5%). Conclusions: Despite a positive attitude toward transplantation, the willingness to donate is low. This may be due to cultural, informational, and emotional factors. Educational and cultural strategies are needed for chronic patients and the general population to encourage an informed decision and promote donation.
