Arrhythmogenic right ventricular cardiomyopathy is a condition associated with severe ventricular arrhythmias and risk of sudden death, making early diagnosis essential. We present the case of a 17-year-old female athlete in whom the combination of clinical, electrocardiographic, and imaging findings enabled a non-invasive diagnosis. Echocardiography showed right ventricular dilation with hypertrabeculation, and cardiac magnetic resonance revealed extensive transmural non-ischemic late gadolinium enhancement. This case highlights the diagnostic value of characteristic imaging features and the importance of risk stratification for considering an implantable cardioverter defibrillator as primary or secondary prevention.
