Cholesterol granuloma of the maxillary sinus is a rare condition characterized by a granulomatous reaction to cholesterol crystals, usually by hemorrhages in the paranasal sinuses. Although it is most common in the middle ear, it also occurs in other areas. Its symptoms are nonspecific, such as nasal obstruction, facial pain and headache. Diagnosis is made by imaging studies and histopathological confirmation. The treatment of choice is surgical resection. We present the case of a 54-year-old patient with a history of endoscopic surgery, whose final diagnosis was cholesterol granuloma and a dentigerous cyst, without recurrence.
