Neonatal urinoma: a safety valve? Case report

Neonatal urinoma is a perirenal collection secondary to disruption of the collecting system, associated with severe congenital urinary obstruction. It is considered a leakage mechanism that reduces intraluminal pressure and preserves renal function. Although it is usually associated with posterior urethral valves (PUV), it can also occur in other types of obstruction. We present the case of a premature neonate with urinary retention, renal dysfunction, and dilation of the urinary tract. Cystourethrography revealed left grade V vesicoureteral reflux, urinoma, and suspected PUV, which were not confirmed by cystoscopy. The neonate had a favorable outcome with spontaneous resolution. Subsequent studies showed left ureterovesical stenosis and probable ectopic ureteral insertion.
