Objective: To evaluate clinical outcomes and patient satisfaction following penile girth enhancement with autologous fat. Method: An observational, prospective, and analytical study was conducted between July 2024 and July 2025 in 48 adult patients who underwent penile girth enhancement with autologous fat. Flaccid penile circumference was measured using a flexible measuring tape preoperatively and at 1, 6, and 12 months postoperatively. Satisfaction was assessed using a Likert scale (1-5). The primary variables analyzed were circumference increase and patient satisfaction. Data were obtained from systematically recorded medical charts stored in an anonymized digital database. Descriptive statistics and Spearman’s correlation analysis were applied, considering p < 0.05 statistically significant. Results: The mean age was 41.19 ± 11.59 years. Baseline circumference was 8.01 ± 0.48 cm and reached 10.45 ± 1.16 cm at 12 months, with a mean increase of 2.44 ± 1.03 cm (p < 0.001). Age showed a negative correlation with total increase (? = ?0.669; p < 0.001) and a positive correlation with fat resorption (? = 0.506; p = 0.0002). Satisfaction demonstrated a strong positive correlation with the achieved increase (? = 0.720; p < 0.001) and a negative correlation with age (? = ?0.472; p = 0.0007). The complication rate was 8.3%, predominantly mild. Conclusions: Penile girth enhancement with autologous fat is an effective and safe procedure, achieving a clinically significant increase in circumference and high patient satisfaction. Age influences fat resorption and the magnitude of the outcome, constituting a relevant predictive factor in clinical practice.
