Superusers of troponin in the fourth level of care: understanding the clinical profile and patterns of troponin request in a veteran’s hospital

Introduction: Troponin is an essential test for the study of patients with chest pain. Few studies have documented the patterns of request and its overuse. Objective: To determine clinical characteristics, diagnoses, and troponin request patterns, in patients treated at a veteran’s hospital, between January 1, 2022, and June 30, 2022. Methods: Retrospective cohort of patients with request for troponin. Number of requests, troponin results, complementary diagnostic studies were reviewed. Descriptive analysis, summary and dispersion measures for continuous variables, absolute and relative frequencies for categorical variables were performed. Results: 4,036 requests in 2456 patients, 21.4% more than one troponin. Sample size 343 patients. Median 66 years, men 55%, indication for troponin request of cardiovascular origin 42.9%, first troponin positive 30.9%, second troponin positive 67.1%, echocardiogram request 39.8%. Conclusions: The largest proportion of patients in whom troponin was requested was not due to a cardiovascular diagnosis, explained by the presence of patients with Covid-19. The largest proportion of patients received a single troponin evaluation. The most frequently requested study after taking troponin was the echocardiogram.
