Objective: To describe the health-related quality of life (HRQoL) in adult solid organ transplant recipients in Argentina. Materials and methods: A cross-sectional observational study was conducted (May 2023 - June 2024). The study included 361 adult transplant recipients. The EQ-5D-3L questionnaire was used. The analysis included stratification by organ type and time windows post-transplantation. Results: Kidney transplantation was the most frequent (66%; n = 240). 49% (n = 177) reported no impairment in any dimension. The most affected domains were anxiety/depression (36.8%) and pain/discomfort (24.9%). The median VAS score was 82 (IQR: 70–90). HRQoL remained stable across time window analyses (median utility: 0.924 in all groups). Kidney recipients reported greater mobility impairment than liver recipients (18% vs. 8%; p = 0.042). Compared to the general population, the transplant group showed higher impairment in anxiety/depression and lower impairment in pain/discomfort. Conclusions: Transplantation is associated with satisfactory and stable HRQoL levels in the long term. Mental health was identified as a priority area for comprehensive clinical follow-up. The originality of this study lies in the subgroup analysis and the provision of local evidence.
