2. To request permission for other reuse content

-Provide full details of the material that you would like to use (journal title, publication year, volume, issue, page numbers) and details of the material that you would like to use (abstract, figure(s) or table(s), excerpts, other (please state)

-Type of use, please select for which purpose you like to use the material (book, journal, newsletter, thesis, slide kit, promotional brochure, classroom materials, poster, CD-DVD, USB, TV/Video, training material, iPads/Tablets/App, other (please state) -How and in what format will the material be used (print, E-versions); quantity (number of print copies); for E-versions (estimated number of users –if not open access-, estimated of numbers of downloads), for conferences, traning lecture, etc (estimate number of participants)
